Regulations and agreement for processing and storing personal data
We appreciate your trust and want to inform you about how we process and protect your personal data received through the contact form and/or when booking an appointment online. We ask for your explicit consent to collect and use the following data.

However, in alcohol and drug-related services, your identity information may be verified against your ID so that the report, if necessary or desired, can be used as evidence in a legal context or in court proceedings.
- Date of Birth: Specifically your year of birth, which contains medical information for analyzing and determining age-specific diagnoses.
- E-mail address: To notify you of your appointment and to have the result provided in a PDF report by the AZ Delta clinical laboratory.
- Phone Number: To contact you in case of failure to keep scheduled appointments.
In alcohol and drug-related services or other medico-legal investigations, the results of which may be used as evidence in a legal context or in court proceedings, the following data may also be collected and processed to verify your identity:
- Name and first name: Will be checked against your ID.
- Date of birth: Will be checked against your ID.
- National Register Number: Verified against your ID for unambiguous identification of your identity.
2. For what purpose do we collect this data?
We use this data for the following purposes:
- Processing and delivering your request: To record, manage your request and ensure that you receive the requested services correctly.
- Status Updates and Communication: To keep you updated on the progress of your application and to communicate important information related to your application.
3. How long do we keep your data?
Betamedics NV retains your data for at least 6 months to achieve the above purposes and in accordance with legal obligations. After this period, your data will be destroyed in order to protect your data.
AZ Delta Clinical Laboratory retains your data and samples for as long as necessary to achieve the above purposes and in accordance with legal obligations.
Betamedics NV is not responsible for the protection or destruction of your data when it reaches the AZ Delta clinical laboratory.

5. Blood analysis and results.
Your blood will be analyzed in AZ Delta’s clinical laboratories. The results are evaluated analytically by AZ Delta clinical biologists and provided with an overall conclusion on your test report each time.
If there are abnormal results that require further medical review, you will be advised to contact your primary care physician or treating specialist via this general decision. The AZ Delta clinical laboratory is not responsible for further medical follow-up or treatment advice.
Your results will be treated with the utmost confidentiality and will never be shared through electronic patient records at AZ Delta or other hospitals. Your blood samples will be destroyed after a minimum retention period.
Your blood draw will be performed by experienced and qualified laboratory staff. As a result of the blood draw, some people may become delirious in the head or tend to faint. Report this feeling to our staff member immediately. The site of the puncture may be painful. Very rarely, there may be post-bleeding, especially in people who take blood-thinning medication or have a clotting disorder. If you have a known coagulation disorder or take blood thinning medication, please report this before the blood collection. If the puncture site does bleed, press the wound well and contact a physician.
6. Cost of provisions
All provisions are at the patient’s expense and do not benefit from reimbursement from the NIHDI. The patient confirms being informed of the exact cost of the provisions. The patient agrees to make payment for these determinations at the time of blood collection.

The Supplier excludes, to the extent legally possible, any liability other than the one provided for in these General Terms and Conditions so that the Customer cannot sue and/or involve the Supplier and/or its representatives and/or auxiliary persons (such as there include directors, employees, subcontractors, or any other appointees) extra-contractually in any possible dispute regarding the formation, performance, interpretation and/or termination of any agreement between the Supplier and the Customer.
Customer acknowledges and agrees that, to the extent permitted by law, it may only sue the Supplier and not the representatives or agents of the Supplier (such as, inter alia, its directors, employees, subcontractors, or any other appointees) in connection with the formation, execution, interpretation and/or termination of any validly entered into and then existing contract between the Supplier and Customer. To the extent permitted by law, Customer hereby expressly acknowledges and accepts the exclusion of the application of Article 6.3, § 2 of Book 6 of the Civil Code.
Reciproque clause
The Parties acknowledge and undertake that, to the extent permitted by law, they can only hold each other liable and not their respective representatives or auxiliary persons (such as therein, among others, directors, employees, subcontractors, or any other appointees) within the framework of the formation, execution, interpretation and/or termination of any agreement validly entered into and then existing between the Parties. To the extent permitted by law, the Parties hereby expressly acknowledge and accept the exclusion of the application of Article 6.3, § 2 of Book 6 of the Civil Code.
