Fatigue is one of the most common complaints when first consulting a doctor. It is a sign that your body or mind is under stress. Often the cause of fatigue is easy to pinpoint, such as acute fatigue after an infection.

When the fatigue lasts longer than 1 month and there is no immediate identifiable reason for it, there may be more to it. Before addressing mental fatigue, of course, we must be sure that nothing physical is going on.

You must be sober for the blood test. This means that from 8 p.m. on the evening before the blood test, you should not eat anything and only drink water.
A urine sample is also required.
You will receive the complete report after about 10 days.

How do I prepare?

The optimal time for blood collection is inthe morning after bed rest and overnight fasting. Fasting means not eating and drinking only water after 10 p.m. the night before.

Not being sober mainly affects the determination of substances resorbed from the diet as such (glucose, triglycerides, etc.). Indirectly, lipemic samples (elevated triglycerides) can technically interfere with the determinations. A number of parameters (e.g., iron, phosphate, cortisol) also show fluctuations during the day.

Depending on the type of study, midstream urine must be produced or first-voided urine:

Collection of midstream urine sample: for ordinary urinalysis

The examination requires the middle portion of the (best first morning) urine, called midstream urine.

For women:

  • Beforehand, wash the pubic area with a washcloth with water:
  • Spread the labia and clean with water (no soap!) or with a damp cloth (always from front to back) and dry with toilet paper.
  • Keep the labia spread when urinating.

For men:

  • Pull the foreskin back and clean with water(no soap!) and dry with toilet paper.
  • Hold the foreskin back when urinating
  • First pee a little in the toilet and then collect some urine in a clean urine jar by holding it in the urinal.
  • Then close the jar tightly with the lid.
  • If necessary, write your name and date of birth on the jar and store it in the refrigerator.

Collection of“first void” urine sample (for STD testing):

First void urine means first discharged urine (first void = first 10 to max. 20 ml). Patients should not have urinated in the last two hours before collection (so preferably morning urine = first urine when getting up). Only the first 20 ml should be collected. The rest of the urine is further urinated out in the toilet.

If first-void morning urine is not possible, urine may also be made on site provided no urination has occurred in the last 2 hours. This first-void urine (only the first 20 ml of the urine stream; rest back out in toilet) should be collected in a sterile urine container.

Through this extensive test panel, we will look for the most common causes of physical fatigue that you can objectify in the blood. Finding the cause is very important because most of the time you can easily intervene through dietary adjustments, supplements or medication.

The panel measures more than 30 key parameters:

  • glucose (sober)

  • HbA1c

  • creatinine

  • eGFR

  • CRP

  • GGT

  • HGB

  • MCV, MCH, MCHC

  • Red blood cells

  • Hematocrit

  • White blood cells (WBC).

  • Thrombocytes = platelets

  • WBC formula

  • TSH

  • Calcium

  • Magnesium

  • Iron

  • Iron saturation

  • Ferritin

  • Albumin

  • Urine Screening

  • Vitamin B12

  • Vitamin D

  • CMV IgG

  • CMV IgM

  • EBV IgG

  • EBV IgM

  • EBNA

  • anti-tTG IgA

  • Deaminated Gliadin IgG

  • Lyme IgG

  • Lyme IgM

Allows detection of the most common physical causes of abnormal fatigue.
A non-exhaustive overview:

  • Diabetes

  • Decreased kidney function

  • Bacterial infection: broad screening via CRP

  • Viral infections: both recent and past infections with cytomegalovirus (CMV) and Epstein-Barr virus (EBV, also known as mononucleosis or glandular fever) are detected

  • Tick-bite disease caused by Borrelia burgdorfi (Lyme disease)

  • Gluten intolerance or celiac disease: common food intolerance

  • Vitamin deficiency: analysis focusing on the most common vitamin B12 and D deficiencies. When these are deficient, it often indicates deficiencies of other vitamins as well. Thus, these act as the canary in the mine.

  • Iron deficiency: especially in growing children, women menstruating. Iron deficiency can also be a sign of invisible blood loss through the stool in duodenal cancer or stomach ulcers, or absorption problems

  • Thyroid problems: both too low and too high thyroid hormone production causes fatigue that can be misunderstood for a long time.

  • Blood cell analysis of both red and white blood cells: impaired blood formation is a direct cause of fatigue and is itself caused by many underlying conditions. Thus, it is a good way to detect treatable diseases.

  • Normal liver function

All tests are conducted in collaboration with doctors and specialists from AZ Delta.
